Published in the European Journal of Pharmaceutics and Biopharmaceutics, the study highlights that the clinical environment often dictates success more than the drug formulation itself. Researchers from Indonesia, China, and Japan evaluated several delivery methods, including mucoadhesive films, sprays, and hydrogels, finding that each serves a distinct purpose. While flat lesions respond well to films, irregular tumors require injectable gels that conform to complex tissue surfaces.

The review emphasizes that oral conditions are dynamic, particularly during radiotherapy or chemotherapy when side effects like xerostomia alter salivary flow and mucosal integrity. These physiological shifts directly impact how drugs adhere to and release within the cavity. Professor Hasan stresses that future development must prioritize predictability, calling for testing protocols that account for enzymatic activity, mechanical stress, and pH levels. Only by aligning delivery systems with the realities of the oral environment can researchers improve clinical outcomes for patients suffering from oral squamous cell carcinoma.
